All-on-4 is one of the most quietly important developments in modern implant dentistry — and also one of the most over-promised. It deserves an honest explanation, because it changes lives when used appropriately, and creates problems when it is not.
What All-on-4 actually is
All-on-4 is a full-arch restoration in which a fixed bridge of teeth is supported by four carefully positioned implants in a single jaw. The two posterior implants are angled to make the most of available bone, which often allows treatment without bone grafting. It is a full-arch solution, not a single-tooth solution, and not a replacement for healthy individual teeth.
Who it suits — and who it does not
All-on-4 is the right answer for patients who have lost most or all of their teeth in a jaw, whose remaining teeth cannot be saved, or who have struggled for years with loose, painful or failing dentures. It is not the right answer for patients whose own healthy teeth could still be preserved. A serious clinician will sometimes talk you out of it — that conversation is one of the strongest signs that you are in the right place.
The staged timeline
A typical international All-on-4 journey is delivered in two visits. The first visit handles examination, planning, surgery and a fixed temporary bridge, usually across five to seven days. After three to six months of healing, a second visit places the final bridge, refined for fit, bite and aesthetics. There are no shortcuts that improve on this timeline — only shortcuts that compromise it.
Materials, costs and what should be included
A serious quote should name the implant brand, the temporary and final bridge materials, the number of clinical visits, imaging, the senior clinician responsible, transfers, and follow-up. It should also be honest about what is not included. If a quote is dramatically lower than its peers, the difference is almost always in one of these places.
Looking after a full-arch restoration
A well-made All-on-4 restoration can last for many years, but it is not maintenance-free. Daily cleaning, professional hygiene visits, and a clinic that remains contactable long after you have flown home all matter. The clinics that take this seriously tend to be the same ones that took your planning seriously in the first place.
